IMPORTANT ANNOUNCEMENT from the Perlmutter Health Center
Effective April 1st, 2004, the Perlmutter Health Center will no longer provide Medicare covered services. We are happy to continue providing medical services to all of our current Medicare patient, but after April 1st, these services will not be covered under Medicare.
Our office will continue to provide prescription services to our Medicare patients for a period of 60 days, until May 31st, 2004. Please make a note of this date. In addition, we would be pleased to provide copies of medical records for any of our Medicare patients who choose to transfer their care to another medical practice.
All of the current services provided by our center will be continued and in fact, expanded. These services will not be covered under Medicare, but will be available to everyone.
